  • GLP-1 Medication and Night-Time Food Noise

    For a while it was quiet.

    That is how nearly everyone describes it. The constant background
    negotiation about food — what is in the cupboard, what you will have
    later, whether you already had too much — went down in volume, and the
    quiet was startling. Some people say it was the first time in thirty
    years their head was not talking to them about food.

    Then, at some point, the evenings started talking again.

    Or a particular stretch of the week started talking again. Nine
    o’clock. The chair. After the house goes quiet. And the first thought
    most people have is: it is coming back, and it must be me.

    Let me deal with the disclaimers first, because they matter here more
    than usual.

    Where my lane ends

    I am a hypnotist and a hypnotherapist. I am not a doctor, and I do
    not treat any condition.

    Anything to do with your medication — how much, how often, whether to
    continue, what is happening in your body, what any symptom means —
    belongs between you and your prescribing clinician. Not between you and
    a blog post. If the change you are noticing has you concerned, that is a
    phone call to your doctor, and it is a reasonable one to make.

    What I work on is the mind side. The habits, the hour, the story you
    tell yourself at nine at night. That is a real and separate thing, and
    it is what the rest of this is about.

    Two different circuits

    Before hypnosis I spent years as an electrician, and this is the
    frame I keep coming back to.

    Run two loads on one circuit. One of them hums loudly enough that you
    never notice the other. Take that load off and the panel is not silent —
    you can suddenly hear the second one, and it sounds new even though it
    has been running the whole time.

    Appetite and habit are not the same thing. They ride together for
    years, which is exactly why nobody separates them.

    A habit is a learned sequence. Cue, action, relief. It is built by
    repetition and it does not check in with your stomach before it fires.
    It was there long before the medication and it does not know anything
    about the medication.

    So when the appetite side gets quieter, a lot of people expect the
    whole thing to go quiet. When the evenings start up again, it feels like
    backsliding.

    It usually is not. It is the second load, now audible.

    Why the evening specifically

    Look at what an evening actually is.

    It is the same hour, in the same room, in the same chair, usually
    with the same show or the same phone in your hand, at the end of a day
    in which you held it together for eight or ten hours.

    That is not an appetite. That is a cue stack — time of day, location,
    posture, screen, and the finish line feeling of a day you got through.
    Every one of those has been paired with eating hundreds or thousands of
    times.

    The mind is a pattern machine. Line the cues up and the sequence
    tries to run. It is doing what it was built to do, on schedule, whether
    or not your body has any interest.

    Some people describe the same thing arriving late in a dosing
    interval rather than late in the day. Same principle, different clock:
    the volume changes, and what was underneath becomes audible.

    Pattern, not willpower

    I want to be very plain about this, because it is where people beat
    themselves up.

    If the chatter returns at nine o’clock, that is not evidence that you
    are weak, that you are wasting your treatment, or that you were always
    going to fail at this. A learned sequence firing on cue says nothing
    about your character. It says the sequence was well learned.

    Willpower is a poor tool here anyway. It is a finite thing you spend
    all day, and by nine at night you have already spent it on work and
    children and traffic and other people’s moods. Asking the depleted end
    of your day to out-muscle a pattern with ten thousand repetitions behind
    it is not a plan.

    You do not out-muscle a pattern. You give it something else to
    do.

    Replace, do not delete

    A mind will not accept a vacuum. Take a nightly routine away and
    leave a hole, and the hole will get filled by the strongest available
    candidate, which is the thing you just removed.

    So the work is a swap, and it is small and specific.

    Start by naming the actual sequence, in detail, in daylight. Not “I
    snack at night.” The real thing: I finish loading the dishwasher, I
    sit in the left corner of the couch, I pick up the remote, and the
    thought arrives before the show does.

    Now you have something to work with, because you know where the seam
    is. The seam is almost never the eating. It is the sit-down.

    Then give the hour a job. Not a punishment and not a project —
    something that occupies the same slot and the same hands. Tea made
    slowly and deliberately. A different chair. Ten minutes outside. The
    dull, physical, unimpressive kind of thing that no one would ever
    describe as inspiring.

    That is what hypnosis is genuinely good for, by the way. Not magic
    and not motivation. It is rehearsal. It lets you run the new sequence,
    in the right order, often enough that it starts arriving on its own
    instead of needing to be summoned. Repetition is what built the old
    pattern, and repetition is what builds the replacement.

    If what you are feeling is real physical hunger rather than the
    pattern talking, that is a question for you and your clinician, and not
    one I will touch.

    The sleep piece

    One more thing, because these two subjects sit next to each
    other.

    A short night makes an evening harder to steer. Most people know this
    from their own experience — tired evenings are the ones where the chair
    wins. Sleep is not a weight-loss intervention and I am not presenting it
    as one. It is simply the difference between meeting nine o’clock with
    some steering, or meeting it with none.

    If you snore heavily, gasp or stop breathing during the night, or
    fall asleep during the day without meaning to, please see a doctor. Some
    sleep problems are physical and belong with a physician.
